Documents Needed:
Appeal Letter- requesting a reduced rent is required and must include a declarative statement (i.e. “I cannot afford the quoted Fair Market Rent in letter.”)
1. Notarized or
2. Ensure the following is included or it will NOT be accepted: “I hereby declare under penalty of perjury that the foregoing is true and correct.”
3. Ensure the letter includes: printed applicant name, signature, date, current address, current phone number and disaster information: DR4393NC
Supporting Documentation: Required as per when submitting an appeal in writing
Note: Pre & Post Acceptable Documents: can be acquired from utility companies, Social Security office, Insurance company, etc.
a. Pre acceptable months (June, July, & August of 2018)
b. Post acceptable months (January 2020)
c. If unattainable, a self-declarative statement can be included in appeal letter
Income- for all family members over 18 years old
1. Current pay stubs
2. W-2 or most recent tax return
3. Documentation of government assistance including Social Security, etc.
4. Unemployment documentation
5. Self-employed documents
6. Unemployed with no income: a self-declarative statement can be included in appeal letter
Utility Bills
1. Pre and Post disaster mortgage (Homeowners, Flood and Taxes if included)
2. Pre and Post Homeowners, flood or taxes if not included in the mortgage
3. Pre and Post disaster water bill
4. Pre and Post disaster electric bill
5. Pre and Post disaster sewer
6. Pre and Post disaster gas (not for cars)
7. Pre and Post oil bill
8. Pre and Post disaster trash
9. Pre and Post disaster propane
10. Rent for housing unit (if they rented short term)
